Privacy Notice March2018

From time to time, medical and lay visitors to our practice may need to inspect our medical records for quality and educational purposes. Even though such inspections are carried out under our supervision, if you would prefer that your notes are not inspected, please let us know in writing.

Viewing Your Records

You have the right to know what information we hold about you.  If you would like to see your records, please speak to the practice manager.  You may also be asked to bring photographic identification to your appointment.

Our guidelines and A Subject Access Request (SAR) form (final two pages) is available here: Access to Health Records

On-line Access to Your Records

We now have the facility for patients to have on-line access to view, export or print information relating to medications, allergies and adverse reactions. This is known as your "summary information".  Please ask at reception for access details.  You will need photographic identification and you will only be given access to your own record.

 Sharing Data

Anonymised patient information is used at local and national level to assist the Government to plan services.  If you do not wish your information to be used in such a way, please inform us in writing.
